Perfect for what we needed to do! It has conditions and and you can really customize your forms in several ways! I had a little issue with some conditons but support was quick at fixing it!

We have been using this app for over a year and have built several forms with it. The app is easy for our team to use. No app is perfect, and this app has some limitations. However, it's nice that the customer service is quick to respond when we ask for help.
