We used the Pify Form on our shop, and are very happy with how the form works. All we needed was a simple form with radio buttons, text boxes and dropdown selections. This app has provided it all and more! Customer service is excellent too. Got a very quick respond after sending an email and the issue was solved in a matter of minutes. Would definitely recommend it.
The only app that allows image multi choice and rules based form construction. If you work with international clients or product images to assist in selection it's perfect.

Excellent app! lots of great features that are all incredible easy to set up. Form looks great and lends a professional touch to any website especially as you can personalise the look and appearance to suit your branding. Good support too - quick to respond and resolve.

So far enjoying using the form builder, I am hoping to be able to make use of the free plan for our needs, but not sure if that is going to be possible yet.
