Great app, easy to install, cost effective, helps cover some liability. Does not add box on checkout due to shopify's restrictions with 3rd parties changing the checkout settings. You can prevent this issue by removing buy now options, and requiring your customer to order through your cart.

We've used this app for a couple years with no issues.. We installed another app that had somehow made the checkbox intermittent. I contacted the team through the chat on the app and they replied within minutes, looked into it and needed collaborator access. I approved the request, and the issue was fixed in less than 5 minutes!! Thank you Efffective Apps team!
