Great experience. I have had this app installed on one site for about a year and have had no problems.
I installed it on another site and got hung up. Their support answered my request almost immediately late on a Friday. That was unexpected, but super awesome. They told me what I did wrong and offered to copy over the forms from the existing site to the new site.

I had issues at first, however the staff was extremely responsive and helped with providing the solution I needed to make the form work exactly how I wanted within the store!
