One of the best apps you could find on shopify which works exactly as per your requirement, very customizable and precise. GREAT support team Their support team is way better compared to the platform's support team, they're always ready to provide help in any possible way, They helped me out when Shopify told me "it's beyond their capabilities" which is ridiculous honestly.

Really easy form builder - I recently migrated from WordPress where I had a lot of flexibility and I was concerned that Shopify would be a bit restrictive but this app shows me that it can be as flexible as WP!
