Nice, it working for me. But there should be a text field which will accept amount from user.
I used this app to embed a customer survey on my shopify store. Super easy to use with lots of good functionality. Way more than I expected for a free app! Glad I found this!

SO simple, easy, and intuitive to use. Can't believe (yet so grateful) that this app is free! I'm excited to see all the different ways I can use the Form Builder app.
I do NOT like this app I don't like that it is not from my URL and that it requires folks to create an account. People should be able to ask questions seamlessly.
