I would give this app more than 5* if I could but I cant. The app is great & the staff over on the online chat no what they are doing. I really didnt no what i was doing and i was ready to chuck the towel in but a lovely guy called sanjay has helped me through setting up the app even when its been something very silly. If it wasnt for him i dont think id be using the app. Anything i have asked him to help with he has done it. Thanks so much. the guy needs a raise :-) The app really does what its intended for. I went straight in with the paid version and I ant not lie it is the best around.

Super useful plugin, even in the free version!

The app allows me to have a mixture of check boxes as well as text boxes which is helpful. they were super supportive to help me fix how the box was sitting on the page as well within a short time frame. great app!
