Works as it should. Decent number of customize options. The "X" to close the popup needs to be moved to the outside of the popup or closer to the corner as it is too close to the title copy area.
We got sued because the pop up is not compliment for people with disablilities that require screen readers
Forms are not sending notifications every time a form is submitted, it is sporadic and causes us to miss requests.
The app is user-friendly but lacks advanced features. It would be great to have the ability to scale the popup's dimensions and configure a dismissal window of 14 to 30 days to improve the user experience.
For the most part it is easy to use and successful at managing registrations. An update that we'd find helpful would be creating some sort of visual status tracking for form submissions (such as highlight/checkmark) - currently they all look the same and stay visible even after they have been completed. Currently we have to click in to the customer to see the tag we use to indicate completion.
We’ve been using Shopify Forms successfully for a long time, but since February 21 we’ve encountered a serious issue that’s impacting our order processing.
When a customer first signs up via a form (where only first name and email are collected), Shopify creates a customer profile without a last name. Later, when that same customer places an order and enters their full name at checkout, the order contains the correct full name for shipping — but the original customer profile, what is showing in the order tab, is not updated with the last name.
This creates major downstream issues for integrations (in our case, our ERP system), because many systems rely on the customer profile data rather than the order address. As a result, orders fail to sync due to missing last names.
This behavior did not cause issues for us before, so it appears something may have changed recently in how customer data is handled at your side.
For businesses that rely on external systems, this is not a small inconvenience — it directly affects order processing and operations.
We hope this can be addressed quickly, as it forces merchants to add friction (like requiring last name for newsletter signups) just to avoid system failures.

App doesn't notify you when you get a form submission. I found a wholesaler request FOUR MONTHS after they'd tried to reach out.
Finding out that Shopify Forms has allowed adding a weird looking "claim with Shop" button on top of the form without notifying us really killed the trust in this app.
Overall OK! A bit flimsy to find submissions, this should be simplified and there should be a way to delete spam or nuisance submissions. Also it should request customer to fill in both their first and last name. I'd be happy to give 5 stars in future if these issues are fixed.
It's made capturing emails SO MUCH EASIER.
Two areas of improvement:
- Sometimes it takes too long to load, losing potential leads
- Missing button customization, as it's clashing with the rest of the store's look and feel
Otherwise, a 10/10! Must have.
