Powerful Contact Form Builder

The UI for building the form can feel tricky at times, but once you get the hang of it, it can probably do (almost) exactly what you need.

I had quite specific niche requirements and it works great.

The support is also brilliant, Taylor and the team were able to help fix some formatting challenges I had at short notice.

This is a great form app, we have been using it really happily for 2 years. But I have only needed customer support now, when bots have started to use it to register in huge amounts; Emma was fast to amend my settings, but also showed me how in case I needed to do the same on other forms. Brilliant service, thank you.

The app has all the features I need and when I have had trouble with something due to my own knowledge limitations, Taylor from support has been prompt and very helpful to figure it out. Had some issues with forms not coming in immediately after filling in. After contacting them through the in-app chat, Taylor helped us out and got the issue resolved within the hour. We've been using this app for our customer service quieries and it has been great! Totally recommend.

We've been using this app for years and there was an issue with our site and the form had to be readded to the page it was on. I was having trouble with it and reached out to support. Quietly broke the integration that sent form submissions to our gmail & no response from their support in 72 hours.

This app does it all! And if something extra is needed, the friendly support team will quickly make it happen.

Here are some examples of what makes this app and support team outstanding:

1) I did not want the form to have a "tab stop" on labels, so the support team added a snippet to my Shopify theme to remove them.

2) After the form is submitted, it gets initialized. But the checkbox's "unchecked event" was not running which left a conditional element (file upload) visible when it should not have been. The support team fixed this right away, and now the form displays the correct elements after form submission and initialization.

3) Custom smtp allows emails to be sent through our own domain resulting in professional email communication. As a result, the emails will always have a high quality score which helps to keep them from going to spam.

4) All elements can be styled using the apps "custom css" feature, or by updating the Shopify theme's css file (base.css).

Note that we are using the Premium plan, which provide us with way more than we currently need but with room to grow as needed.
