Great app for my business. The forms fields are saved in the customers' profiles, making it very easy to use the data.
Also, updating my website theme messed around with the coding of the forms (it didn't show up on the site anymore,) so I reached out to customer service who had the issue resolved in less than an hour.

super easy and great variation of form elements, The email you receive after it has been filled out could be a little better (uploaded photos a bit larger)

Very happy with the app and excellent customer service second to none. Despite me having a million questions they were more than happy to help many thanks.
