We have found this app very valuable in obtaining qualitative post purchase customer feedback. There are many question types and basic styling is possible. Jason is super responsive when we have any questions. I recommend this app.

Zigpoll is awesome. I needed a specific update for my form panels (checkbox randomization) and the founder actually made sure it was built and implemented for me within a week.
